STABLE INTRAOPERATIVE PULSE OXIMETRY VALUES DO NOT PROTECT AGAINST A DECREASE IN REGIONAL CEREBRAL OXYGEN SATURATION IN PATIENTS UNDERGOING PRIMARY HIP ARTHROPLASTY. A PILOT STUDY

چکیده

Background and Study objective: Continuous perioperative monitoring of blood oxygen saturation via pulse oximetry is an established standard care. The incidence postoperative cognitive dysfunction (POCD) in orthopedic patients higher when compared to any other group hospitalized patients. Although its etiology remains unclear, factors such as thromboembolic complications may play a role. Thromboembolic material, including that released from the surgery site, enters bloodstream then travels brain. decrease flow involved cerebral vessels saturation. Considering this, we decided determine if regional monitored by near-infrared spectroscopy corresponds with readings who underwent primary total hip arthroplasty. Methods: A 20 scheduled for elective arthroplasty were enrolled study. All procedures performed under spinal anesthesia intravenous sedation. Monitoring (rSO2) was carried out time initiation until end surgery, using method. In all, 15 analyzed. Results Conclusions: We found following: (1) rSO2 values decreased continuously over (2) value changes not related mean arterial pressure variations or hemoglobin analyzed oximetry.

Pulse Oximetry Overestimates Oxygen Saturation in COPD.

BACKGROUND Measurement of oxygen saturation with a handheld pulse oximeter is widely practiced as a surrogate to invasive arterial blood gas analysis. Oxygen saturation is an important parameter in cases of COPD, but there are insufficient data on the role of pulse oximetry in patients with COPD, moreso in diseases across its spectrum, such as chronic bronchitis and emphysema. We assessed the p...

Impact of extracranial contamination on regional cerebral oxygen saturation: a comparison of three cerebral oximetry technologies.

BACKGROUND Cerebral oximetry is a noninvasive technology using near-infrared spectroscopy (NIRS) to estimate regional cerebral oxygen saturation. Although NIRS cerebral oximetry is being increasingly used in many clinical settings, interdevice technologic differences suggest potential variation in the ability to accurately acquire brain oxygenation signals. The primary objective of this study w...

Analgesic efficacy and safety of epidural oxycodone in patients undergoing total hip arthroplasty: a pilot study

BACKGROUND AND OBJECTIVES Oxycodone is poorly studied as an adjuvant to central blockades. The aim of this pilot study was to assess the efficacy and safety of oxycodone hydrochloride in epidural blockade among patients undergoing total hip arthroplasty (THA). PATIENTS AND METHODS In 11 patients (American Society of Anesthesiologists physical status classification system II/III, age range: 59...

Cerebral oximetry: a replacement for pulse oximetry?

Cerebral oximetry has been around for some 3 decades but has had a somewhat checkered history regarding application and reliability. More recently several monitors have been approved in the United States and elsewhere and the technique is emerging as a useful tool for assessing not only adequate cerebral oxygenation but also tissue oxygenation and perfusion in other organs.
